Has anyone experienced overheating Triacs causing melting of the casing and /or fires?
it might be a loose connection to the heat sink.

I would initially question your load type in particular the input capacitance.
The LE dimmers should not be loaded with more that 300nF per channel.
The effect of too high input capacitance when dimmed produces a high current spike when the triac is switched ON. This happens 100 times per sec.
